AGREEMENT: The following agreement between Door To Door Driving School LLC (hereafter known as "Driving School"), the student driver (hereafter known as "Student") and the sponsoring adult/Parent (hearafter known as "Sponsor") if any, whose signature is signified by submitting this form, shall consist of the following charges and obligations:
1. For Teen Drivers: It is agreed that tuition of $375.00 will be charged and paid in full at registration for classroom instruction (15 2-hour instructor led sessions) and behind the wheel instruction (6 2-hour behind the wheel sessions each to include 1-hour of behind the wheel guided practice and 1-hour of driver observation).

1. For Teen Drivers: It is agreed that tuition of $375.00, for nine private drive sessions will be paid in full at the first drive session.
AGREEMENT: The following agreement between Door To Door Driving School LLC (hereafter known as "Driving School"), the student driver (hereafter known as "Student"), whose signature is signified by submitting this form, shall consist of the following charges and obligations:
1. For Adult Drivers: It is agreed that tuition of $60.00 per hour will be charged for each hour of private behind the wheel instruction. The number of hours of private instruction will be ; The total tuition fee of $ will be charged and paid in full at registration.
2. The Driving School will not refund any tuition or part of tuition if the School is ready, willing, and able to fulfill its part of the agreement.
3. All behind the wheel instruction will be given in a vehicle approved by the Wisconsin Department of Transportation and equipped with a dual control foot brake and a second inside rear view mirror.
4. Student must obtain instructional permit ("Temps") before behind the wheel instruction sessions can begin. The Student will be required to present his/her temps at each behind the wheel session.
5. A certificate of completion, which must be presented to the DMV road examiner at the time of the road test, will be given to the Student at the completion of all classroom instruction and behind the wheel drive sessions.
6. If the Student should miss an upcoming scheduled drive with less than 24 hours notice to their drive instructor, a $35 fee will be assessed.
This constitutes the entire agreement between the Driving School and the Student/Sponsor, and no verbal statement or promises will be recognized.
